With the Fed efile 'form' open, there should be a tab at the bottom. It's toward the right and you might have to widen the window for it to come into view.

It's the 11th tab at the bottom, so the easy way to find it is to click on the First tab, 'Pages & Worksheets', and then select it from the list.
