Does anyone have a QB data transfer authorization template

I have a new client that have had their bookkeeping done by another accountant in town and she wants to get something in writing signed by the client before she gives me a backup of QB.  Which is totally reasonable.  But - being lazy here - does anyone have a basic letter that they use?

NATP used to have sample letters for information release but they may have mostly related to the release of tax information and you would have to be a member of NATP to access them from their website.  And I truthfully don't know if they are still available on the site, so I really like Abby's suggestion best.

If the other accountant wants something in writing , they should provide the release document
