Form 1045 (review help)

Would anybody here be able to review a form 1045 (2 years) for me?  It's my first time preparing one and want to make sure I am preparing correct, anyone (PM me)?  I prepared a sample returns, no names or SS# are being used, just the numbers are real.

I’ve always had better luck amending the prior return instead of using 1045.
