Attach note to state return

In ATX, how doe you attach a note to a state return that will be e-filed?

The statements/elections form? There's a blank statement you can use. If the federal return goes with the state (in CA there's a checkbox for if any schedules other than A or B are attached, you check it and a copy of fed goes with state).
