How do you get the form 8889 to be for spouse. I have box 12b check W with a amount on W-2. But the 8889 only list the taxpayer name and number and I do have the W-2 marked as spouse.

There is a check box for spouse somewhere on 8889, I think on one of the input sheets. (Don't have it in front of me at the moment.) You can actually have 2 8889's in a return, if each spouse has their own HSA.
