CA 5805 -how do you request a waiver?

I have been all over that form and I can't find a place to enter the information to request a waiver.

help please.

Yeah, I see that, but where do you go to mark the box? I can't find it.

Sorry about my first post. It did not explain my problem very well. I see the check box, but there is no way to click on it without doing an override. I know I am missing something very simple, but I just can't see it.

Look at the bottom of the input page. where you would choose the page of the form after 1,2,3 "line 1 Part1 explanation"

Will that do it for you. I don't have one needing a waiver so I can't check it out.

In an earlier post from me - I really hate these things. Too many clients have no idea what I am even asking when forms or information is missing. Grrr....
