I received a spreadsheet from my BNA rep which lists all states, their filing requirements, due dates and requirements for filing extensions. If this works, I have attached the spreadsheet.

I figured it out. There was a button right in front of me that said "attach this file". DUH.

Update for Louisiana extensions - Louisiana will NO longer accept the federal extension in lieu of a Louisiana extension. LA form 2868 is required for individuals; LA 620-EXT is required for corporations. Louisiana also requires that all extensions be electronically filed, either on the LDR website or via third party tax software.

LA has some nerve forcing all taxpayers to file the extensions electronically! Do all LA taxpayers have access to internet and own a computer? I never heard of a state forcing the taxpayer to file electronically if they don't want to.
