1099B with “final principal payment”

Ok I have a 1099B with three transactions on it and;

Date of Acquisition not listed

Cost or other basis is not listed

Gain or loss not listed

The first transaction list a cost basis factor; so I can use that for cost.

The second two in the additional information shows “final Principal payment”

Not sure how to account for this on Sch. D
